Personal health information is only disclosed with your consent. Your authorization to disclose personal health information may be withdrawn in writing at any time.
Personal health information is protected and steps are taken to safeguard your information. Records are retained for a period of ten years and disposed of as required by the College of Physiotherapists of Ontario.
You may obtain access to your health record or request correction of your record by contacting Elizabeth Crawford (Physiotherapist).
Complaints may be addressed to Elizabeth Crawford (Physiotherapist), or to the Information and Privacy Commissioner as per the Personal Health Information Protection Act, 2004.
